*Verify Coverage*
To determine your eligibility and coverage details, please email a clear copy of your insurance card to FiveElementCare@gmail.com. Ensure the email includes your contact information including phone number and date of birth.
*Insurance Verification Process*
- Our team will promptly verify your insurance coverage and contact you with the details. Please allow us some time to complete this process. If you need immediate service, we usually have same day services for Self Pay Clients. Visit our Services Page for Details or call our office Directly.
*Schedule an Appointment*
- Once your coverage is confirmed, our friendly staff will reach out to you to schedule an appointment at your convenience.
